Observed the following improper dispensing utensils: A small plate and bowl were left in the food item in the top food area of the prep table. Provide for all food items be dispensed by something with the scoop. If the bowls are used to measure the food, they must be stored outside of the food items to avoid contamination. Immediate/Onsite
14  Observed the following improper food contact surfaces: 1) Top of a small stand table used to hold seasonings and such had the table top portion duct taped in areas, 2)Ice machine lid is held together with duct tape and is cracked and splintering along the bottom lip. 15  Observed the following improper non-food contact surfaces: 1)Bottom of the to-go sauce cooler by front counter is rusty, 2) Shelf above the ice bin for the soda machine is chipping.
